If you are able to download and fill out this form prior to your visit it will help with time management.  If you are unable to do so please arrive 10 minutes early to your appointment.

YOU MUST ONLY FILL OUT 1 FORM

If you are coming for an assessment of your allergies please pick the form that most resembles your symptoms.  If you have hay fever type symptoms please choose Allergies(sinus).  If you have an upset stomach, vomiting or diarrhea please fill out Gastro-Int symptoms.  If you have any issue with your skin please fill out Urticatia (rash).  If you think you have a latex allergy please fill out the Latex form.  Please make sure you have followed the allegy patient instructions prior to your visit.

If you are coming for cardiac evaluation or general internal medicine please fill out the New Patient Registration form.

All patients are kindly asked to confirm their appointment 1 week ahead at 604-870-3277.  If we do not hear from you we will cancel the appointment and offer it to someone on our cancellation list.  If you miss a confimed appointment there is a $100 charge.

Do you have a cancellation list?

Yes! and we encourage you to add your name if your availability is flexible. Sometimes Dr Gill will open days and we can bring you in much sooner.

Do you charge a no show fee?

We try not to.  We hope you can attend your appointment.  If you do not confirm your appointment we cancel you and offer the time to another patient.  Only if you confirm the appointment and then no show do we charge you.  There are different fees depending on the type of appointment you miss.  If you need to cancel kindly give us 24hrs notice.

Why do I need a referral to see Dr Gill?

Dr Gill is a specialist.  As per Medical Services Plan all specialist appointments must be requested by your family doctor.  Your referral is good for 6 months from your first visit.  If you need to be seen again you will need to get another referral. 

What languages does Dr Gill speak?

Dr Gill is fluent in English, Punjabi, Hindi and Urdu.

Where should I park?

There is parking on site.  If the parking lot is full, please park at Mill Lake.

Why should I come early to my appointment?

There is a patient registration form that needs to be filled out.  We may also run an electrocardiogram for you depending on your symptoms.

Why do I need to bring my medications?

This is very important!  Dr Gill may need to add or change your medications.  Some of your medications may also interfer with tests Dr Gill wants to order for you.

What services does Dr Gill offer in office?

Pacemaker interrogation, Holter monitor, Blood pressure monitoring, Event recorders, Diabetic teaching with an RN, Stress testing, ECGs, Allergy testing, Food Challenge.
